Student-Athlete Media Release and Questionnaire
The Mercyhurst College Office of Athletic Communications keeps a file of your athletic history, including the Student-Athlete Media Release, and will make available that information to the media and professional scouts if given permission.
If you are an incoming student-athlete (freshman), transfer student-athlete OR a current student-athlete who wishes to make changes to your profile, you must complete the Student-Athlete Media Release.
The Family Educational Rights and Privacy Act of 1974 requires that Mercyhurst College receives your consent to disseminate information contained in your student file. By submitting the Student-Athlete Media Release, you give the Mercyhurst College Athletic Communications office permission to release the information in your file. Your consent also gives the office permission to nominate you for honors pertaining to academic and athletic success. Also, the Buckley Amendment requires us to ask that you agree to this release form.
Please be assured that your file is confidential and the only people who are allowed to view it are the director of athletic communications and his/her staff. By submitting the Student-Athlete Media Release, you authorize the Mercyhurst College Athletic Communications office to review and disseminate any information in your educational/athletic records to third parties for general public relations purposes.
Occasionally, the office may need to make use of additional information collected and/or maintained by the College for public relations purposes. Information obtained from your records will assist with compilation of personal information for use in athletic department publications (brochures, programs, press releases,etc.).
